As we prepare to begin the Winter 2021-22 sports season, please be reminded that student athletes must be tested for COVID-19 weekly in order to participate unless they are fully vaccinated. This policy follows the recommendations outlined by the CDC and NYS Education Department and is intended to protect the health and safety of the school community. 

If your student plans to participate in a winter sport and is unvaccinated, please complete this testing consent form (PDF) and return it to the Health Office as soon as possible (see below).

If your student athlete is fully vaccinated against COVID-19, please submit proof of vaccination to the Health Office (see below). If your student athlete is in the process of getting vaccinated, please complete the testing consent form (PDF) for now and then submit the proof of vaccination when you have it.

The Health Office requests that consent forms and/or vaccination records be submitted to both school nurses via email at sbrown@cairodurham.org and spebler@cairodurham.org. You can submit a scanned copy of the vaccination record or send a photo attachment. You may also fax items to 518-622-2948 however email is preferred. All medical records will remain confidential. 

While vaccination is not required to participate in winter sports, it is strongly encouraged to help further reduce the risk of virus transmission. If you are interested in getting your child vaccinated, click here to find nearby locations offering the free Pfizer vaccine, which is the only vaccine currently authorized for people 12-17 years old. There are options in Greene, Columbia, and Albany Counties. Fully vaccinated student athletes are not required to undergo weekly COVID-19 testing.
