Daily Health Screening Reminder

Please be reminded that we need your help reporting daily health screening information for your child(ren) every morning before sending them to school for in-person instruction. Please submit the online screening form, which will be sent to the primary contact via email, by the following times each morning:

  • Middle/High School (6-12) - submit by 6:45 a.m.
  • Elementary School (PK-5) - submit by 8 a.m.

If you are unable to submit the form each morning by the deadlines noted above, you must call the main office of your child's school as soon as possible so we can be prepared to screen the student(s) upon arrival:

  • Elementary School: 518-622-3231
  • Middle School: 518-622-0490
  • High School: 518-622-8543

The daily health screening form applies only to students who are participating in in-person instruction. You do not need to complete the daily form on remote instruction days (Grades 6-12) or for students who are participating in fully remote instruction.

This mandatory daily health screening is a new state requirement to help ensure the health and safety of our school community. If you have any questions or concerns, or if you need to confirm your contact information, please contact the appropriate main office. We appreciate your understanding and cooperation.

How the Daily Health Screening Works

We will be using the secure platform EduForm (you may need to check your spam folder for a message with the subject "Daily Health Screening Entry" and allow EduForm as an approved contact). The primary contact will receive an email each morning with links to complete the brief health screening questionnaire for your child(ren) before sending them to school. The daily health screening asks Yes/No questions about recent travel, COVID exposure and temperature (e.g., does the student have a temperature of 100.0 degrees Fahrenheit or greater).
